Evaluation & Treatment Rates
Initial Evaluation (60–75 min) — $300
A comprehensive assessment, to include a history review, your current concerns, and treatment goals.
Follow-Up Appointment (20–30 min) — $125
Medication management, progress review, and adjustments to your treatment plan.
Pre-payment Discount
A 10% discount will be offered if a patient pays for four visits in advance.
What to Expect
Before Your Visit:
Complete the intake forms sent through email, text, or our secure portal.
Gather relevant medical or mental health records.
During Your Visit:
Your provider will review your history, goals, symptoms, and create a customized treatment plan.
After Your Visit:
Access your notes and follow-up instructions through the patient portal.
Schedule follow-up appointments online anytime.
